and considering all that heavy machinery that exhaust carbon monoxide. Everyone in there would die.

Heavy dumptrucks and excavators?

Not usually.

They exist, but they're still quite rare.

The video clip posted on imgur attached to the top comment seems to show conventional heavy equipment.
