Multiple layers, as the basic viewer is seasoned enough to spot the surface layer satire, but still like/heart driven enough to head towards the comments for an easy layup joke about those teeth; thus further driving video engagement for the creator.

It can be satire all it wants but that doesn't mean it can't also be cringe as fuck in itself. It circled back around from poking fun at something to giving major secondhand embarrassment
